Things You Need to Know

What to Bring

  •  Your insurance card & photo ID 
  •  Script or Referral, if required by your insurance

What to Expect

When you first walk through our office door, you will be greeted by one of our support staff. Then, you will be given paperwork to fill out before seeing your therapist, if you have not done it online prior.


  • Demographic Form
  • Medical History & Medications
  • Office Policy & HIPPA Form
  • Outcome Measurement Test Questionnaire
  • Any other forms required by your specific insurance company


 You will then be evaluated by one of our licensed Physical Therapists. This evaluation will include, but not be limited to:


  • Checking your range of motion
  • Strength & Reflexes 
  • Muscle flexibility of the affected & surrounding areas
  • Specific special tests to help diagnosis your pain

Based on these findings, your Physical Therapist will develop with you a plan of care to “help you feel like you again”. The plan of care may include modalities such as:

  • Ultrasound
  • Electric Stimulation
  • Heat/Ice
  • Traction
  • Neuromuscular Re-education
  • Gait Training 
  • Hands on techniques of massage, mobilizations, stretching, myofascial release
  • Exercise Program for in office & home

Everything is all tailored to your specific needs

You may experience some level of soreness following the evaluation, which is normal, as we determine the cause of your pain or problem to better treat you.

The first visit will last approximately 45 minutes to 1 hour, with subsequent visits averaging around 40-45 minutes (depending on your plan of care).

Things to Know

  • Your evaluation will be performed by a licensed Physical Therapist, not by a Physical Therapist Assistant as they are not fully trained to perform the evaluation and develop a diagnosis and treatment plan. 
  • DO YOUR EXERCISES! Research shows that most patients respond much better when clinical therapy is combined with home exercises. 
  • EMBRACE YOUR THERAPY GOALS! Your Physical Therapist’s intent is to help you regain your independence, educate you about your condition, share self-treatment methods, and emphasize a strong wellness component for injury prevention in the future. 
  • To ensure strong communication, always ask your Physical Therapist any questions you have regarding your care. 
  • Always feel free to contact the front office staff for questions regarding your health insurance or payment issues.
